Before any surgical procedure, we require this consent form to be completed. It provides important details about your pet’s procedure, safety measures, and pain management protocols, ensuring you feel confident in your decision.

Please complete all required sections with accurate contact and medical information and read the surgical care details carefully before signing. If you have any questions, please call us prior to your pet’s scheduled procedure.

Dear Client,

Your pet is scheduled for an upcoming surgery. To help make the surgery day as smooth and stress-free as possible, we have prepared a few simple forms for you to review and sign in advance. Along with this letter, the following forms are available on our website for your review:

  • Pre-Anesthetic Blood Testing Information Form
  • Surgical Information Sheet

Please review all provided forms carefully. If you have any questions, feel free to contact the hospital by phone at (613) 822-6009 or by email at info@rsah.ca. On the day of your pet’s surgery, you will be asked to review and sign an Authorization/Estimate Form. We will also confirm a phone number where you can be reached during your pet’s procedure.

The night before your pet’s surgery, please do not provide any food or treats after 10:00 pm. Water is permitted and may be left out for your pet. If your pet is currently receiving any medications, vitamins, or injections, please withhold the morning doses unless otherwise instructed by the doctor.

At the time of your pet’s scheduled surgery, the doctor will address any remaining questions or concerns and will collect the completed Authorization Form.

If you have chosen to proceed with any recommended blood tests, these will be completed prior to the start of surgery if they have not already been performed. Should any questions arise, the doctor may contact you using the phone number provided on the Authorization Form.

Once the surgery is complete, the doctor will process your payment, review all discharge instructions verbally, and provide you with a written copy. If you do not understand any instructions, please do not hesitate to ask for clarification before leaving the hospital or afterwards.
